Nurse Manager - Sub-Acute
ALIYA of Oak Lawn is seeking an experienced and dedicated Nurse Manager to lead clinical operations within our sub-acute program. The Nurse Manager is responsible for overseeing daily nursing operations, ensuring high-quality resident care, supporting regulatory compliance, and promoting positive clinical outcomes for residents requiring short-term rehabilitation, complex medical care, and post-acute services.

- Oversee the daily clinical operations of the Sub-Acute Unit
- Ensure delivery of high-quality nursing care and positive resident outcomes
- Supervise, mentor, and support nursing staff, including RNs, LPNs, and CNAs
- Conduct clinical rounds and monitor resident conditions to ensure appropriate interventions are implemented
- Collaborate with physicians, nurse practitioners, therapy staff, and interdisciplinary team members to coordinate care
- Ensure compliance with facility policies, CMS regulations, IDPH requirements, and nursing standards of practice
- Assist with admissions, transfers, and discharges for sub-acute residents
- Monitor documentation for accuracy, completeness, and regulatory compliance
- Participate in care plan meetings and interdisciplinary team discussions
- Assist with infection prevention, quality assurance, and performance improvement initiatives
- Investigate incidents, accidents, and resident concerns and implement corrective actions as needed
- Support staff education, competency validation, and ongoing clinical development
- Monitor staffing needs and assist with scheduling to ensure appropriate coverage
- Promote a culture of accountability, teamwork, and resident-centered care
- Perform other duties as assigned by the Director of Nursing
- Graduate of an accredited nursing program
- Current, active Registered Nurse (RN) license in the State of Illinois required
- Minimum of 2 years of nursing leadership experience in a skilled nursing, rehabilitation, sub-acute, or long-term care setting
- Strong knowledge of post-acute care, clinical operations, and regulatory compliance
- Experience managing medically complex residents, IV therapy, wound care, and rehabilitation services preferred
- Excellent leadership, communication, and organizational skills
- Proficient in electronic medical records and clinical documentation systems
- Ability to effectively lead teams and manage multiple priorities in a fast-paced environment
