REGISTERED NURSE – HOME HEALTH

Location:

• This Home Health RN position serves patients throughout Chicago's North Side and Lakefront communities. The territory includes neighborhoods from Rogers Park and Edgewater south through Uptown, Lakeview, Lincoln Park, and Downtown Chicago, with travel concentrated along the I-90 corridor and surrounding communities. Nurses manage a geographically aligned caseload, providing in-home care to patients across a diverse urban service area.

Schedule: This position includes a rotating schedule with weekend, holiday, and on-call responsibilities. Team members work every third weekend and participate in an on-call rotation approximately three times per month on weekdays (5:00 PM – 8:00 AM) . Additional on-call coverage is required every sixth scheduled working weekend . Holiday requirements include one winter holiday and one summer holiday each year .

Sign-On Bonus Available – Up to $10,000!
This position is eligible for up to a $10,000 sign-on bonus. You must be actively working in this same position at the time of payout. Current internal team members are not eligible at this time.

Major Responsibilities:

• Nurse/Patient Relationship:
  Develops and maintains a therapeutic relationship throughout the healthcare continuum.

  • Demonstrates adaptive communication styles based on individual patient needs

  • Provides individualized care to promote physical and psychosocial comfort

  • Assesses patient and family readiness and involves them in planning and implementing care. Seeks consultation as needed.

  Facilitates the patient and family’s right to receive quality, cost effective care.

  • Mobilizes appropriate resources in response to situations that have the potential to negatively impact patient and family outcomes

  • Demonstrates ability to critique own nursing practice and achieves required certifications or Bachelor’s Degree
