Home Health RN Case Manager, North

This Home Health RN position serves patients throughout Chicago's North Side and Lakefront communities. The territory includes neighborhoods from Rogers Park and Edgewater south through Uptown, Lakeview, Lincoln Park, and Downtown Chicago, with travel concentrated along the I-90 corridor and surrounding communities. Nurses manage a geographically aligned caseload, providing in-home care to patients across a diverse urban service area.

This position includes a rotating schedule with weekend, holiday, and on‑call responsibilities. Team members work every third weekend and participate in an on‑call rotation approximately three times per month on weekdays (5:00 PM – 8:00 AM). Additional on‑call coverage is required every sixth scheduled working weekend
. Holiday requirements include one winter holiday and one summer holiday each year

Sign‑On Bonus Available – Up to $10,000! This position is eligible for up to a $10,000 sign‑on bonus. You must be actively working in this same position at the time of payout. Current internal team members are not eligible at this time.

  • Earns a minimum of 10 contact hours per year

• Registered Nurse (RN)
• A valid driver’s license issued by the Division of Motor Vehicles.

• Graduate from an accredited school of nursing
• Earn 10 continuing education credits annually
• One year of medical surgical experience
• Has demonstrated proficiency for the requirements for RN role

• Good communication and interpersonal skills

• Familiarity with patient care equipment
• Current Illinois RN license
• Current CPR
