LVN LPN

Belmont Village Oak Park is seeking a compassionate LVN/LPN to provide high-quality nursing care to residents in our assisted living and memory care community. In this resident-centered role, you'll administer medications, monitor health status, support care plans, and respond to changes in condition. You'll collaborate closely with caregivers, nurses, and families to ensure safety, dignity, and comfort for seniors. Responsibilities

• Provide direct nursing care to assisted living and memory care residents
• Administer medications and treatments according to physician orders and facility protocols
• Monitor residents' health status and promptly report changes to supervising nurse or physician
• Document care, observations, and interventions accurately in medical records
• Assist with development and implementation of individualized care plans
• Collaborate with caregivers, nurses, and other team members to ensure coordinated care
• Educate residents and families on care plans, safety, and wellness practices
• Support activities of daily living and promote residents' dignity, comfort, and independence
• Follow all safety, infection control, and regulatory guidelines
• Participate in ongoing training and quality improvement initiatives
