You will guide patients and families through a pivotal transition with clarity, empathy, and confidence, conducting assessments to determine hospice eligibility and educating families about Medicare benefits.
This on-site role in Oak Park, IL offers a Monday–Friday schedule with on-call weekends, and an hourly pay range of $42–$46.
