A healthcare consulting firm in the Chicagoland Area is seeking an individual to support the IL Medicaid Care Coordination team. Responsibilities include outreaching to IL Medicaid members, verifying that waiver services are initiated in a timely manner, and assisting with health risk screenings. Candidates should have a Bachelor's degree or equivalent experience in health care, familiarity with medical terminology, and proficiency in Microsoft Office.

This role offers a contract duration of 5 - 6 months and involves telephonic and work-from-home arrangements with occasional onsite meetings.
