Physical Therapist (PT) – Home Health Case Manager
Up to $10,000 Sign-On Bonus
Schedule & Coverage Area- Schedule:
Day shift - Primary Coverage Area:
South Suburban Chicago communities, Country Club Hills, Flossmoor, Homewood, Markham, Hazel Crest and East Hazel Crest
This position is eligible for a sign‑on bonus of up to $10,000, subject to applicable taxes. The employee must remain in this position at the time of payout. Current internal team members are not eligible.
Position HighlightsPhysical Therapists with Advocate at Home enjoy the autonomy of managing their own caseload while being supported by a reputable, stable, and values‑driven organization. As part of Advocate’s integrated healthcare system, Home Health Physical Therapists work independently while collaborating closely with physicians, nurses, and interdisciplinary team members.
As a Home Health Case Manager, the Physical Therapist establishes meaningful relationships with patients and families while assessing and addressing mobility, safety, communication, and durable medical equipment needs. This role offers the opportunity to make a direct impact by helping patients safely recover and regain independence in the comfort of their homes.
What You Need- Licensed Physical Therapist in the State of Illinois (or eligible for licensure)
- Minimum of one (1) year of Physical Therapy clinical experience, or completion of diverse and comprehensive clinical affiliations
- Proficiency in the use of home-based therapy equipment and assistive devices (e.g., bands, weights, Hoyer lifts)
- Basic computer skills, including Microsoft Word and email; ability to learn electronic scheduling and documentation systems
- Strong verbal, written, and documentation skills
- Excellent interpersonal, time management, and self‑organization skills
- Travel extensively within assigned geographic boundaries in all weather conditions
- Work rotating weekends and holidays based on patient needs and assigned workload
- Attend mandatory informational, educational, and branch meetings
- Assess, plan, implement, and evaluate physical therapy treatment programs for home health patients
- Facilitate patient progression toward optimal levels of self‑care and functional independence
This role requires the full range of body motion, including:
- Bending, sitting, and standing
- Climbing stairs while carrying objects
- Pushing, pulling, reaching, lifting, and carrying a minimum of 25 pounds
- Assisting with patient ambulation on flat surfaces and stairs
- Providing treatment on standard beds in the home environment
Ability to lift up to 35 pounds without assistance. For patient lifts exceeding 35 pounds or when the patient cannot assist, appropriate patient‑handling equipment and additional staff support are expected when available. Unique situations will be evaluated on a case‑by‑case basis.
