JOB SUMMARY

The Project HEAL Case Manager Positions are integral parts of the Community Based Services Program within Claratel Behavioral Health. These positions are a collaborative partnership between Claratel Behavioral Health and other community partners, to support individuals transitioning from hospital and acute care settings into Transitional Housing. These positions will focus on coordinating safe, clinically‑informed transitions from hospitals into residential housing while ensuring continuity of medical, behavioral health, and substance use treatment services.

The Project HEAL Case Managers will provide a combination of community transition, planning, case management, and psychosocial rehabilitation services that bridge medical, behavioral, housing and social services systems. Their work helps ensure clients receive coordinated, wrap‑around care that promotes stabilization, recovery, and long‑term housing success.

DUTIES AND RESPONSIBILITIES
  • Coordinate discharge planning with hospital or acute care providers and medical home/home health services for identified individuals.
  • Conduct transition planning to support movement from facility‑based care into residential settings.
  • Facilitate enrollment and linkage to Claratel Behavioral outpatient services; including counseling, psychiatric services, and substance use treatment.
  • Coordinate care with residential providers, medical providers, behavioral health clinicians and community services agencies.
  • Assist participants with appointment scheduling, transportation planning, benefits linkage, and service navigation.
  • Monitor engagement in treatment and supportive services into housing.
  • Participate in multidisciplinary case coordination with Project HEAL partners.
  • Support continuity of care during the critical stabilization period following discharge.
  • Maintain documentation consistent with Claratel Behavioral Health Policies and applicable regulations.
  • Complete and maintain appropriate data entry into the HMIS (also known as the Client Track) System.
  • Other duties as directed by Program Manager and/or Director.
MINIMUM QUALIFICATIONS
  • High school diploma or GED.
  • At least one year of experience in a behavioral health setting providing case management/community support services.
  • Basic computer and typing skills.
  • Care coordination skills to assist individuals with accessing/linkage to additional resources and services within the agency and/or community.
  • Transportation to and from work.
PREFERRED QUALIFICATIONS
  • Bachelor’s degree in a related field (e.g., human services, counseling, criminal justice, or social work).
  • Two years of experience in a behavioral health setting providing case management/community support services.
NEEDED ATTRIBUTES
  • Excellent interpersonal, problem‑solving, and critical‑thinking abilities.
  • Ability to remain calm in crisis/mental health situations and respond appropriately.
  • Commitment to confidentiality.
  • Professional and effective communication with Community Partners, Stakeholders, Staff, Clients’ families/significant others, and the Public.
WORKING CONDITIONS

The Project HEAL Case Managers will work full time (40 hrs). The positions require evening, weekend or adjusted hours due to program and community needs. Ability to use a computer, telephone, fax, copy, and scanning machine. Must complete/pass agency‑sponsored defensive driving course as well as all other agency‑required training.
