You will independently connect patients and families with long-term care resources, manage transitions, and educate healthcare providers on psychosocial factors impacting patient care.
Qualified candidates must have a Bachelor's Degree in Social Work, LBSW license in Texas, and at least 1 year of social work experience. This role is intended for residents in the Greater Houston area.
