Population Health Management Project Role
This is a key role within the Population Health Management (PHM) project. The role will focus on the near term in California on a groundbreaking data platform for Medicaid that has been selected to deliver, sponsored by the state of California. The ideal candidate has experience with managed care plans, population health management, and has familiarity working with the Department of Healthcare Services (DHCS) in California.
Requirements:
- Bachelor's degree or equivalent combination of education and experience.
- 5 years of working experience in managing large-scale change or transformation initiatives experience working with or at Managed Care Plans, with relevant knowledge of data, quality, and population health platforms.
- Demonstrated knowledge of the Department of Healthcare Services (DHCS) and their programs and initiatives for the Medi-Cal population.
- Experience working with the interface of data and functional groups within an organization. Excellent presentation skills, and the ability to communicate complex technical concepts simply and effectively to all audiences.
- Build relationships and facilitates an on-going dialogue with the target stakeholder groups on PHM service and ways the service could better achieve its goals of improving Medi-Cal members' health and experience by offering a whole-system, person-centered service.
- Engages to surface unmet needs map and document opportunities for the PHM service to meet those needs.
- Provides input into developing and modifying systems to meet stakeholder needs and develops business specifications to support these modifications. Participates in technical reviews and inspections to be carried out through the project.
- Develops recommendations to drive utilization and proficiency with the PHM Service, that increase benefit realization, and the achievement of results and outcomes tied to the wider PHM-CalAIM program.
- Hybrid work environment 1-2 days a week in the office as needed for face-to-face and client meetings with the remainder of time remote work. The office is located in the West Sacramento area.
- Specifically looking for someone with knowledge of the CA Medicaid landscape (MCP and BHP policy expertise).
