OASIS Coordinator (OASIS Review, Coding & QA)

The OASIS Coordinator is responsible for reviewing, coding, and quality-assuring OASIS assessments in Axxess to ensure clinical accuracy, regulatory compliance, and appropriate reimbursement. This role supports clinicians in accurate documentation and works closely with clinical and billing teams to ensure timely, compliant submission of OASIS data. This is a hybrid position requiring regular in-office collaboration combined with remote work flexibility.
Key Responsibilities- Review completed OASIS assessments in Axxess for accuracy, completeness, and internal consistency
- Assign and validate ICD-10-CM diagnosis codes in accordance with home health coding guidelines
- Perform quality assurance (QA) review of clinical documentation within Axxess to ensure compliance with CMS regulations and agency policy
- Identify and communicate documentation gaps or clinical inconsistencies to field clinicians for correction, routed through Axxess workflows
- Ensure timely completion and submission of OASIS data within CMS-required time frames, tracked via Axxess QA and scheduling tools
- Monitor case-mix and outcome data using Axxess reporting; flag trends impacting reimbursement or quality scores
- Collaborate with clinicians, case managers, and billing staff to resolve documentation and coding discrepancies
- Stay current on CMS OASIS updates, ICD-10 coding changes, and home health Conditions of Participation
- Maintain accurate QA records within Axxess to support audit-readiness
- Provide education/feedback to clinical staff on documentation and coding best practices
Required:
- Active RN license (or LPN/LVN, depending on state/agency requirements)
- Minimum 2 years of home health clinical experience
- Experience with OASIS assessments (OASIS-D/E) and home health coding
- Hands-on experience with Axxess software (clinical, QA, and/or coding modules)
- Working knowledge of ICD-10-CM coding guidelines as applied to home health
- Familiarity with CMS home health regulations, PDGM, and Conditions of Participation
- Strong attention to detail and analytical skills
- Reliable home internet/workspace setup to support hybrid work arrangement
- HCS-D (Home Care Coding Specialist – Diagnosis) or COS-C (Certificate for OASIS Specialist – Clinical) certification
- Prior experience in a dedicated QA, coding, or utilization review role
- Experience with case-mix optimization and outcome/quality reporting in Axxess
- Excellent written and verbal communication
- Ability to work independently and manage multiple assessments under deadlines
- Strong critical thinking and clinical judgment
- Comfortable providing constructive feedback to clinical staff
