This position offers a unique opportunity to work remotely while providing essential support in a school setting. You will join a multifaceted team of rehabilitation professionals, collaborating closely to design and implement interventions that cater to the specific psychosocial needs of students. The work schedule typically involves regular weekday hours but may occasionally require evening and weekend availability based on student needs and caseloads.

Key Responsibilities
  • Conduct thorough psychosocial assessments through interviews, direct observations, and comprehensive reviews of medical records to discern emotional, social, and environmental factors influencing student recovery.
  • Provide compassionate counseling and emotional support not only to students but also to their families, addressing issues related to adjustment, grief, and overall mental health.
  • Collaborate effectively with the broader rehabilitation team, contributing to the development of individualized treatment plans that holistically address patients' psychosocial challenges.
  • Assist students in navigating community resources, including support groups, vocational training, and housing assistance, enhancing their ability to manage life post-rehabilitation.
  • Educate patients and their families about available community resources and post-rehabilitation services that can aid in their ongoing recovery and adjustment.
  • Play a vital role in discharge planning, ensuring smooth transitions back to home or alternative care settings.
Patient Population / Client Focus

The primary focus of this role will be on school-aged children and adolescents dealing with a variety of challenges stemming from physical or mental health conditions. Your work will significantly impact their recovery journey, facilitating their reintegration into social and academic environments. By addressing their emotional and social needs, you will help them achieve their highest potential and maintain functional independence.

Qualifications
  • Possession of a Master's degree in Social Work from an accredited program.
  • Active state license as a Licensed Clinical Social Worker (LCSW) is required.
  • Deep understanding of psychosocial dynamics, adjustment issues, and coping strategies, particularly in a rehabilitation context.
  • Exceptional communication and interpersonal skills, enabling you to form trusting relationships with students and their families.
Preferred Experience
  • Previous experience working in rehabilitation or medical settings, especially with pediatric populations, is highly preferred.
  • Familiarity with interdisciplinary care models and the integration of psychosocial factors into rehabilitation strategies.
