About the Position:During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

What to Expect:
- Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and documentation according to Matrix guidelines and protocols.
- Collaborate with physicians, case managers, social workers, family members, caregivers, and medical personnel as appropriate.
- Work with Primary Care Physicians on patient education and follow-up.
- Provide services in various venues such as home visits and skilled facility visits.
Type: PRN - Flexible - Nurse Practitioner
Pay: Earn from $2,000 up to $10,000 per month (based on 20-100 visits at $100 each). Compensation varies with availability and assessments completed.
Location: Home, SNF, and other community environments in Elkland, Pennsylvania
Hours: Days, Evenings, Nights, Weekends, Flexible Scheduling
Benefits: PRN providers are non-benefited, except for 401(k) and limited voluntary benefits.
Mileage Reimbursement plan included.
Qualifications:- Master’s Degree or equivalent experience with NP licensure.
- Current RN and NP licensure in the practicing state, including prescriptive authority or ability to obtain it.
- Board certified by AANP, ANCC, or AACCN in an approved specialty.
- Current BLS, ACLS, or CPR certification.
- 1+ year experience as a Nurse Practitioner preferred; new grads are encouraged to apply.
- Strong computer skills and familiarity with medical record software.
- Excellent communication skills with patients and colleagues.
- Flexibility and comfort with change.
- Travel to meet patients where they live is required.
- Valid driver’s license, ability to drive, and proof of adequate auto insurance are necessary.
