Nurse Practitioner

Nurse Practitioner (NP) or Physician Assistant (PA) - Geriatrics/Internal Medicine

Carease Health - NJ is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a hybrid role, this position combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Our company prioritizes proactive measures for enhanced physical health and well‑being, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive preventative care program.

• Provide high-quality preventative care to residents in skilled nursing facilities through on‑site visits and remote work from home
• Conduct assessments, develop treatment plans, and prescribe medications as needed
• Collaborate with healthcare teams to ensure comprehensive care and minimize hospitalizations
• Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to enhance patient outcomes
• Maintain accurate and timely documentation of patient care and outcomes
• Participate in quality improvement initiatives to enhance patient care standards

• Active NP or PA license (in the state of practice)
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
