Pilsen Medical Clinic & Associates is actively seeking an Independent Contractor (APNP) Advanced Practice Nurse Practitioner to complete home visits around the Chicagoland area. We invite you to choose a career with our growing home visiting physician team! As a team we work together to provide primary care services for seniors in their homes. Our goal is to provide seniors with the highest quality medical services in the greater Chicago area, emphasizing wellness and management of chronic diseases.

Each practitioner is partnered with a medical assistant to ensure exceptional, quality care for each patient.

• Provide exemplary customer service to patients, visitors and coworkers.
• Provide convenient, friendly patient care to all seeking health-related services.
• Perform medical assessment of patients.
• Complete home visit Encounter Summary within 48 hours of appointment.
• Assure compliance with licensing, certification, and accrediting bodies.
• Represent company and the clinic at internal and external meetings or events as reasonably required.
• Maintain working equipment and supplies.
• Coordinate patient care with patient's associated home health agency.
• Review patient case documents in a timely manner.

Experience:

• New grads welcome to apply.
• Nurse Practitioner: 1 year (Preferred).

License/Certification:

• Advanced Practice Registered Nurse (APRN) – Required.
• Advanced Practice Registered Nurse (APRN) – Preferred.
• NP – Preferred.

Job Types: Full-time, Part-time

Pay: $60.00 - $80.00 per hour
