Optum Care is seeking a Part-time Nurse Practitioner to join our Home-based Medical Care team in Missouri. The role is 30 hours per week, Monday–Friday, 8am–7pm, delivering home-based care to complex, chronic patients to help older adults live at home.
You will conduct initial assessments, develop care plans, coordinate with PCPs, and perform post-discharge visits. Requires NP certification and active state licensure, plus relevant experience.
