Role DescriptionThe RNCM, Admissions, On Call role is a full-time remote position responsible for coordinating and managing hospice and palliative care admissions. Daily responsibilities include conducting clinical assessments (via telehealth or phone), reviewing medical histories, and collaborating with referring providers to determine eligibility for hospice or palliative services. The role involves explaining care options, services, and admission processes to patients and families, ensuring informed, compassionate decision-making.
The RNCM will triage calls, respond to urgent patient and family needs, and provide on-call support and guidance to field staff to promote continuity of care. This position requires accurate documentation, adherence to regulatory and organizational standards, and ongoing communication with interdisciplinary team members to support individualized care plans.
- Strong Communication skills, including the ability to explain clinical information clearly and provide compassionate support to patients and families.
- Customer Service skills to ensure respectful, responsive, and patient-centered interactions in a remote care environment.
- Sales and Student Recruiting skills applicable to presenting hospice and palliative services, educating communities, and promoting appropriate utilization of care.
- Education skills to effectively teach patients, families, and caregivers about disease processes, symptom management, and available support resources.
- Active Registered Nurse (RN) license in applicable state(s) and eligibility to practice in a remote capacity.
- Previous experience in hospice, palliative care, home health, or related acute/chronic care settings preferred.
- Ability to work independently, manage on-call responsibilities, and prioritize multiple patient needs effectively.
- Proficiency with electronic health records and remote communication tools.
- Demonstrated commitment to holistic, person-centered care and cultural sensitivity.
