Nsight Health is seeking a motivated LVN/LPN to support Chronic Care Management from a remote contact center. You will perform high-volume outbound calls, conduct CCM care plan discussions, and assist with behavioral health integration while documenting interactions in real time.
The role requires an active LPN/LVN license, strong empathy and communication, and comfort using EMRs and telehealth tools. Training covers four weeks, with a 9–6 ET schedule for onboarding and a 11–8 ET shift
