RN Care Navigator​/Case Manager; Fully Remote

• Support patients navigating complex kidney health needs across the continuum of care.

Responsibilities:

• Manage a caseload, coordinate services, and conduct follow-up.
• Educate patients on dialysis, transplant, and treatment options.
• Advocate for patients and document needs, barriers, and goals.

Requirements:

• Active RN license and bachelor's degree required.
• Case management experience and CCM certification preferred.
• Strong communication and problem-solving skills.
• Fully remote position with no travel.
