Job Summary

Case Management at JIREH Workforce Foundation is a collaborative process of assessment, planning, facilitation, coordination, evaluation, outreach, and advocacy designed to meet the comprehensive needs of each participant. The Recruitment and Outreach Specialist (Case Manager) play a central role in managing the participant’s journey from recruitment through graduation, job placement and retention. This includes coordinating barrier reduction services, maintaining regular check-ins, and ensuring that participants are supported academically, professionally, and personally throughout the training program and one year after graduation.

The ideal candidate will demonstrate strong interpersonal communication skills, a deep understanding of community resources, and a commitment to equitable outcomes.

Reports to Executive Program Administrator Duties and Responsibilities Recruitment and Outreach
  • Conduct participant outreach, prescreening and intake interviews with potential participants
  • Conduct educational and service needs assessments to identify individual barriers and strengths
  • Manage application submissions and coordinate with community partners to ensure eligible applicants are enrolled.
  • Host information sessions and orientation. Attend community, networking events and job fairs.
  • Develop Individualized Education Plans (IEPs) for each participant
  • Provide and track individualized supports to help participants overcome non-academic and academic barriers to program completion
  • Provide and track individualized supports to help participants overcome barriers to employment, including transportation, childcare, healthcare, and technology assistance.
  • Coordinate wraparound supports in alignment with the organization’s Barrier Reduction Policy
Student Support
  • Maintain regular contact with participants through weekly check-ins, monthly meetings, and progress tracking.
  • Address academic, personal, and logistical challenges to ensure retention and successful program completion.
  • Facilitate referrals for services including transportation, childcare, healthcare, mental health, housing, and financial literacy
  • Collaborate with instructors and program staff to ensure participant progress is monitored and documented in the CEJA Reporting System
  • Track attendance and academic performance, and coordinate make-up sessions as needed
  • Coordinate 30, 60, and 90-day post-graduation follow-ups and one-year retention tracking
General Programmatic Duties
  • Participate in employment readiness workshops, mock interviews, and resume development sessions
  • Maintain accurate and confidential case files and documentation of participant progress and services provided
  • Participate in interdisciplinary staff meetings and contribute to continuous improvement of program outcomes
  • Stay informed about available community resources and maintain updated referral lists
  • Support equity and inclusion initiatives by using person-first language and promoting a judgment-free environment
  • Perform other duties as assigned by the Executive Program Administrator
Knowledge, Skills, Abilities
  • Strong communication and organizational skills with the ability to manage multiple priorities
  • Knowledge of Chicago community resources and service networks
  • Experience supporting diverse adult learners from underserved communities in an educational or workforce training environment.
  • Proficient in Microsoft Office and data tracking systems
  • Ability to work collaboratively and maintain confidentiality.
Qualifications
  • Bachelor’s degree in human services, Social Work, Psychology, Human Resources, or a related field
  • Minimum of two years of experience in social work, social services, case management, training, or workforce development
Licenses/Certifications
  • Must have valid driver’s license and reliable transportation

  • Salary: $70,000 – 85,000 annually
  • Schedule:

    Monday to Friday, 8:00 am to 4:30 pm (subject to change), occasional weekends as needed for outreach or graduation activities
  • Work Location:

    In person, 1528 W. Adams Street, Chicago, IL 60607
  • 2 remote working days per month**
  • Health (Medical, Dental, Vision) – available after 30 days of work
