Role DescriptionThe Qualified Intellectual Disabilities Professional/SDA is a full-time, on-site role based in Blue Island, IL. This role is responsible for coordinating and overseeing services for individuals with intellectual and developmental disabilities, ensuring that supports are person-centered and aligned with each individual’s goals. Day-to-day tasks include developing and monitoring individualized service plans, documenting progress, and collaborating with interdisciplinary teams, families, and community partners.
The position will fullfill QIDP/SDA responsibilities for individuals at work and home receiving services in Blue Cap's home-based Department.
- BA/BS in social services or related field, plus one-year experience working with people with intellectual disabilities
- Ability to communicate effectively both verbally and in writing
- Proficiency in the use of computers and software applications
- Valid drivers license and good driving record
- Ability to maintain CPR and First Aid certifications
- Ability to complete Department of Human Services QIDP training and background check requirements
