As a Home Health Physical Therapist, you will:

• Conduct comprehensive in-home physical therapy evaluations
• Develop and implement individualized treatment plans based on patient needs and goals
• Provide skilled therapeutic interventions to improve mobility, function, and quality of life
• Monitor patient progress and modify treatment plans as appropriate
• Educate patients and caregivers on exercises, mobility techniques, fall prevention, and home safety
• Maintain timely and accurate clinical documentation in compliance with agency standards
• Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care
• Adhere to all regulatory, clinical, and documentation requirements

• Current Illinois Physical Therapist (PT) license in good standing
• Doctor of Physical Therapy (DPT) degree required
• Home health experience preferred, but not required
• Excellent communication, organization, and documentation skills
• Ability to work independently and manage a flexible schedule
• Professional, dependable, and patient-centered approach to care
• Physical ability to safely perform in-home patient visits
• Valid driver's license, reliable transportation, and current auto insurance
