Nurse Practitioner - Internal Medicine Nurse-Allied

Position Summary

Nurse Practitioner - Internal Medicine physician job in Illinois. We are a growing, physician-owned medical group expanding in Orland Park, IL, seeking an experienced Nurse Practitioner (NP) to join our team in a Skilled Nursing Facility (SNF) setting. This role focuses on post-acute and rehabilitative care, including orthopedic recovery, stroke management, and pain management, working alongside a collaborative lead provider.
This role is weekends only!

• Provide comprehensive medical care to SNF patients, including assessments, diagnosis, and treatment planning.
• Manage patients with orthopedic rehab needs, post-stroke care, and chronic pain conditions.
• Ensure accurate and timely documentation in EMR.

• Active Nurse Practitioner (NP) license in Illinois.
• Active DEA license (or ability to obtain prior to start).
• Minimum 1 year of clinical experience as an APP (SNF, geriatrics, internal medicine, or rehab experience strongly preferred).
• Experience managing adult/geriatric patient populations.
• Strong clinical decision-making and time management skills.
• Comfortable working independently in an SNF setting.

• Skilled Nursing Facility (SNF) or post-acute care experience.
• Background in internal medicine or geriatrics.
• Experience with orthopedic, stroke, or pain management patients.

• Part-Time, Weekends only:
  Saturday and Sunday.
• No nights.

• Prolonged periods of standing, walking, and sitting while working with patients as well as working on a computer.
• Must be able to lift up to 50 pounds.
• May be exposed to blood borne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
• Must wear the required PPE per company and/or facility requirements.
• Must comply with facility vaccination requirements.
