PURPOSE:

The Adult Admissions Coordinator assumes ownership of and responsibility for the intake and admission process for Adult Residential and Adult Day programs at Little City. The Adult Admissions Coordinator is the principle point of contact for residential and day program agency referrals and coordinates the process for assessing and admitting individuals into those Little City programs. Upholds the agency’s core values and mission by striving to ensure that each person has access to the most appropriate and necessary services as well as monitoring performance measurement and compliance which will improve quality of life.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The list of essential duties and responsibilities, as outlined herein, is intended to be representative of the task to be performed. The omission of an essential function does not preclude management from assigning duties not listed herein is such duties are a logical assignment to the position.
1. Manage the intake and admissions process for the Adult residential and Adult Day programs including, but not limited to, fielding phone calls, providing information and resources, answering program questions, providing tours to prospective families, assessing client needs, conducting intake meetings, coordinating intake staffing’s with program staff, as appropriate, and helping to place qualified persons into Little City adult services or programs The primary goal is to fill vacancies with appropriately matched participants.
2. Responsible for the recruitment and filling of residential and day program vacancies in order to maintain census. Develops and works in collaboration with outside referents, placement agencies, school districts, DHS, families, or other agencies.
3. Serve as a liaison between families and/or guardians and program staff from intake through admission.
4. Responsible for ensuring a smooth, efficient admissions process for new participants, and working in collaboration with outside referents, families, placement agencies and agency staff. Provides external and internal support throughout the admissions process.
5. Attends DD/ fairs, expos, work groups and consortiums to represent Little City and all adult program offerings.
6. Lead bi-weekly Intake/Admissions meetings with Adult residential and day programs.
7. Work cooperatively and maintain on-going communication with all programs relative to current and future services offered, current and future program openings and sharing appropriate information and collateral materials.
8. Maintains and updates the referral and intake database/platform with demographics, diagnosis, present placement, and other information as necessary.
9. Develops a process for intake planning from emergency placements to future placements that may be six months to over a year out.
10. Completes other compliance reports or dashboards as requested by program based on monitoring activities completed.
11. Must be proficient in Microsoft Office products, demonstrating advanced skills in creating spreadsheets, tables, formulas and queries.
12. Must be organized and able to manage multiple projects simultaneously.
13. Must demonstrate excellent customer service skills and follow through.
14. Must have working knowledge of Autism Spectrum Disorders, Developmental and Intellectual Disabilities including diagnosis and supportive services.
15. Responsible for all safety activities in your workplace and immediately report all accidents and safety hazards to your supervisor.
16. Maintains American Heart Association certification in First Aid and CPR, if required.
17. Successfully completes Little City required training, re-training, and any additional training as required by Little City policies and practices, or as assigned by his or her immediate supervisor.
18. Other duties as assigned.
