Responsibilities
  • Manage the intake and admissions process for the Adult residential and Adult Day programs, including fielding phone calls, providing information and resources, answering program questions, providing tours to prospective families, assessing client needs, conducting intake meetings, coordinating intake staffing with program staff, and helping to place qualified persons into Little City adult services or programs; the primary goal is to fill vacancies with appropriately matched participants.
  • Recruit and fill residential and day program vacancies to maintain census; develop and collaborate with outside referents, placement agencies, school districts, DHS, families, or other agencies.
  • Serve as a liaison between families and/or guardians and program staff from intake through admission.
  • Ensure a smooth, efficient admissions process for new participants, working with outside referents, families, placement agencies, and agency staff, while providing external and internal support throughout the admissions process.
  • Attend DD/ fairs, expos, work groups and consortiums to represent Little City and all adult program offerings.
  • Lead bi-weekly Intake/Admissions meetings with Adult residential and day programs.
  • Work cooperatively and maintain ongoing communication with all programs regarding current and future services offered, program openings, and share appropriate information and collateral materials.
  • Maintain and update the referral and intake database/platform with demographics, diagnosis, present placement, and other information as necessary.
  • Develop a process for intake planning from emergency placements to future placements that may be six months to over a year out.
  • Complete other compliance reports or dashboards as requested by program based on monitoring activities completed.
  • Be proficient in Microsoft Office products, demonstrating advanced skills in creating spreadsheets, tables, formulas, and queries.
  • Be organized and able to manage multiple projects simultaneously.
  • Demonstrate excellent customer service skills and follow-through.
  • Have working knowledge of Autism Spectrum Disorders, Developmental and Intellectual Disabilities, including diagnosis and supportive services.
  • Be responsible for all safety activities in the workplace and report all accidents and safety hazards to the supervisor immediately.
  • Maintain American Heart Association certification in First Aid and CPR, if required.
  • Successfully complete Little City required training, re-training, and any additional training as required by Little City policies and practices or as assigned by the immediate supervisor.
  • Other duties as assigned.
  • Qualifications
    • Bachelor’s Degree in a Human Services related field (required).
    • At least 2 years of experience working with individuals with developmental/intellectual disabilities (required).
    • QIDP Certification preferred.
    • Excellent verbal and written skills; ability and confidence to give presentations to large groups.
    • Strong interpersonal skills, problem solving abilities, and a positive attitude.

    While performing the duties of this job, the employee is subject to sitting for prolonged periods; frequently walks, stands, grasps, lifts, holds, or feels objects; occasionally stoops, kneels, crouches, or crawls.
