PURPOSE:

To initiate, facilitate, and perform a variety of casework related functions pertaining to children residing in the children’s residential program.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Carries a caseload of children living in assigned homes, who reside in the residential homes on Palatine Campus and in the community.
  • Conducts at least weekly visits with each assigned child in his/her residential home to ensure appropriate services are implemented for the child as well as collaborate with all staff related to the child’s overall care, progress and treatment.
  • Secures documentation, compile historical information, assesses case situations and makes recommendations regarding children on caseload.
  • Collaborates with the interdisciplinary team in developing and implementing the Mental Health Assessment and Individual Treatment Plans for all cases, including quarterly and six-month reports.
  • Attends IEPs and advocates for children in the school system.
  • Attends monthly team meetings for the homes assigned.
  • Monitors and supervises the placements and adjustments of children. Responsible for permanency planning and discharge planning for children.
  • Attempts and maintains regular, ongoing contacts with the parents/guardians of children. For DCFS wards, makes all efforts to establish a relationship with the biological family and assists the biological family in reaching reunification when applicable to the case.
  • Responsible for the upkeep of child’s files which includes obtaining consents, monitoring and completing progress notes, and securing all required documentation for assessment and provision of services to child.
  • When applicable, works in conjunction with the DCFS worker, for implementation and coordination of sibling and birth parents’ visits.
  • Assures that licensing standards and applicable laws, rules, and policies are followed.
  • Assists with the intake process of new children moving into assigned homes, including setting up the child’s file and following up to ensure we receive all necessary documentation.
  • Work cooperatively with other staff in Children’s Services, DCFS, DHS and collateral resources.
  • Responsible for the facilitation of 6-month staffings, as well as, attendance in staff meetings, committee meetings, supervision and trainings as required.
  • Responsible for all safety activities in your workplace and immediately report all accidents and safety hazards to your supervisor.
  • Successfully completes Little City Foundation required training, re-training, and any additional training as required by Little City Foundation policies and practices, or as assigned by his or her immediate supervisor.
  • Other duties as assigned by your supervisor.
