Role Description The Case Manager is a full-time, on-site role based in Palos Heights, IL, responsible for coordinating services and supports for individuals with intellectual and developmental disabilities. On a day-to-day basis, the Case Manager assesses individual needs, develops person-centered care plans, and monitors progress toward goals. This role collaborates with residents, families, interdisciplinary team members, and external service providers to ensure appropriate services, advocacy, and continuity of care.
The Case Manager maintains accurate documentation, supports compliance with regulatory and agency standards, and participates in team meetings and care reviews. The position also involves promoting independence, community integration, and overall well-being in alignment with Bethshan's mission and values.
Qualifications
- Bachelor's degree in a human services field
- Ability to conduct assessments, maintain thorough documentation, and ensure compliance with relevant regulations and agency policies.
- Strong interpersonal, communication, and collaboration skills to work effectively with residents, families, interdisciplinary teams, and community partners.
- Capacity to advocate for individuals' rights, promote person-centered care, and support community integration and quality-of-life outcomes.
- Organizational and time-management skills to handle multiple cases, prioritize tasks, and respond to changing needs.
- Ability to meet any applicable licensing, certification, background check, and health requirements for the role and setting.
