Position: Advanced Practice Nurse, APN, Maine West High, School Based Clinic, Des Plaines - Part Time

Role Overview

Advanced Practice Registered Nurse (APRN) – School-Based Health Clinic. Part‑time, 0.6 FTE, Monday, Wednesday, Friday 7:30AM – 4:00PM at Maine West High School, District
207, 1755S.Wolf Road, Des Plaines, IL60018. This position collaborates with Advocate Health to provide comprehensive primary care within a school setting.

Key Responsibilities
  • Deliver comprehensive primary care services, including acute, chronic, and preventive care.
  • Conduct health assessments, diagnoses, and treatment planning for adolescent patients.
  • Promote health education, wellness, and disease prevention strategies.
  • Collaborate with school staff, families, and healthcare team members to coordinate care.
  • Maintain accurate and compliant medical documentation.
  • Participate in initiatives to expand and enhance school‑based health services.
Clinical Responsibilities
  • Assess, diagnose, and determine or alter treatment and management plans appropriate for age and clinical condition.
  • Order, perform, and interpret diagnostic studies and prescribe pharmacologic and non‑pharmacologic interventions.
  • Manage conditions based on evidence‑based care, cost effectiveness, and risk/benefit assessment.
  • Provide health promotion, disease prevention, and education to patients and families.
  • Work within scope of practice and collaborate with interdisciplinary team; seek consultation or referrals as needed.
  • Escalate need for emergent or specialized care when necessary.
  • Maintain complete, concise, and timely documentation in the electronic medical record and support compliant coding and billing.
  • Facilitate consistent, coordinated care and clear communication among all team members and community agencies.
  • Perform office or hospital procedures in line with specialty practice and granted privileges.
Other Responsibilities
  • Participate in quality, safety, and peer review initiatives; attend organizational and departmental meetings, committees, and work groups.
  • Advise and onboard new team members, students, and health care professionals.
  • Maintain standards of productivity, access, face‑to‑face time, and quality metrics for safe and timely patient care.
  • Continuously develop clinical and professional skills and advance the profession within and outside the organization.
Qualifications
  • Licensed APRN in Illinois, with active national board certification in Family Medicine or Pediatric Primary Care (preferred).
  • Active RN and APRN/APRN‑FPA/APNP or other APRN license(s) in state(s) of practice, and active DEA registration. Illinois Controlled Substance License required prior to hire.
  • Active BLS and/or ACLS, PALS, NRP as required; active Illinois controlled substance license and DEA registration required prior to hire.
  • Master’s Degree in Nursing.
  • Experience in primary care, pediatrics, or school‑based health center preferred.
  • Strong communication, collaboration, and decision‑making skills.
  • Ability to work independently and as an effective member of a health care team.
  • Proficiency with electronic medical record and adapting to evolving technology.
  • Proven organizational skills and ability to prioritize effectively.
  • Physical capability to stand, walk, bend, stoop, twist, and lift up to 50lbs from floor to waist, 20lbs over head, and carry up to 40lbs a reasonable distance.

  • Pay Range: $51.05 - $76.60 per hour.
