As a Licensed Practical Nurse in Hemodialysis, your specialized skill set and ability to collaborate with the Registered Nurse is vital to exceptional patient care.

• Conduct and document a thorough assessment of each patient’s medical status upon admission and throughout the patient’s course of treatment.
• Assist in the implementation of the patient’s treatment plan under the direction of the RN.
• Comply with the evaluation, treatment, and documentation guidelines of the department.
• Act in compliance with hospital, regulatory, and professional standards and policies.
• Maintain clinical currency by pursuing continuing education opportunities.
• Administer IVP medications specific to dialysis patient population.
• Initiate and discontinue dialysis treatments using multiple access sites including AV fistulas, AV grafts, and Central Venous Catheters (CVC).
• Clean and redress CVC skin sites.
• Draw blood from CVC.
• Respond to CVC line separations.

• Graduate from an accredited School of Practical Nursing or hold an ASN/BSN in Nursing.
• Current New York State Licensed Practical Nurse license.
• Complete the NYS Department of Health and NYS DOE approved training curriculum for LPN’s working in Outpatient Hemodialysis Units (as part of unit-based orientation).
• Complete the NYS DOH and NYS DOE approved annual competency for LPN’s accessing central lines in a hemodialysis unit (as part of unit-based orientation).
• Basic Life Support (BLS) Certification required.

One year experience preferred.

Medium work:
Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; requires frequent walking, standing, or squatting. For disease-specific care programs refer to the program-specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.
