We are seeking a compassionate and experienced Social Services Coordinator to join a well-established long-term care and rehabilitation facility.

The Social Services Coordinator is responsible for supporting residents and families through the admission process, care planning, discharge planning, and ongoing psychosocial needs. This role serves as an advocate for residents while collaborating with the interdisciplinary care team to ensure the highest quality of care.

• Conduct psychosocial assessments and develop individualized care plans
• Assist residents and families with adjustment, transition, and discharge planning
• Coordinate community resources and support services
  
  Participate in care plan meetings and interdisciplinary team discussions
• Maintain accurate and timely documentation
• Advocate for resident rights and quality of life initiatives
• Ensure compliance with federal, state, and facility regulations

• Bachelor's degree in Social Work, Human Services, or related field preferred
• Long-term care, skilled nursing, or healthcare experience preferred
• Strong communication, organization, and problem-solving skills
• Ability to build positive relationships with residents, families, and staff
• Knowledge of discharge planning and regulatory requirements a plus
