Position: Registered Nurse (RN) - Home Health - $75K-100K per year

Job Description & Requirements

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Prospect Heights, Illinois. This full‑time position supports patients in Mount Prospect and surrounding areas and is eligible for a $10,000 first‑year success bonus to reward key achievements during your first year.

Responsibilities
  • Lead and collaborate to provide management and delivery of patient care plans as a registered nurse case manager.
  • Support patients in Mount Prospect and surrounding areas.
  • Ensure consistent charting and meet critical deadlines for patient documentation.
Qualifications
  • RN license in the state where you work.
  • Graduate from an approved school of practical nursing.
  • One year of RN experience with the clinical competence to deliver quality patient care.
  • Current driver’s license and ability to spend approximately 20% of your day driving to/from patient locations.
  • Commitment to consistently meet critical deadlines for charting.
  • Skill in self‑managing time and schedule.
  • Demonstrated experience with tablets, mobile phones and EMR software.
