Registered Nurse; RN - Home Health

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Prospect Heights, Illinois.

Deliver skilled, patient‑centered care to patients in their homes within the Buffalo Grove and surrounding areas. This position is eligible for a $10,000 first‑year success bonus for key achievements in the first year of employment.

• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Think critically and solve problems in real time, ensuring thorough & compliant EMR documentation.
• Work independently while collaborating with an interdisciplinary team.
• Travel to patient homes within the assigned territory.

• Active Registered Nurse (RN) license in the state where you work.
• Minimum one year of RN experience with the clinical expertise to deliver quality patient care.
• Current driver’s license and ability to spend approximately 20% of your day driving to and from patient locations.
