As a Restorative Nurse, you will play a vital role in providing high-quality, patient-centered care to our residents. Your expertise will be focused on developing and implementing individualized treatment plans to restore function, independence, and well-being. You will work closely with the interdisciplinary team to identify and address barriers to recovery, utilizing your strong clinical skills and knowledge to promote positive outcomes.

Responsibilities:

• Develop and implement individualized treatment plans to maximize functional ability and independence

• Provide skilled nursing care to restore function, strength, and mobility

• Collaborate with the interdisciplinary team to identify and address barriers to recovery

• Conduct thorough assessments, make accurate diagnoses, and develop effective treatment plans

• Demonstrate exceptional clinical skills, including wound care, medication management, and rehabilitation techniques

• Maintain accurate and detailed records of patient care and treatment plans

• Educate and empower patients, families, and caregivers to promote continuing care and independence

Requirements:
