Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in River Grove, Illinois.

Job Overview

Our full-time position supports patients in Schiller Park and surrounding areas. The role includes management and delivery of patient care plans as a home health registered nurse case manager.

Responsibilities
  • Manage and deliver patient care plans as a home health registered nurse case manager.
Qualifications
  • RN license in the state where you work.
  • Graduate from an approved school of practical nursing.
  • One year of RN experience and the clinical competence to deliver quality patient care.
  • Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
  • Commitment to consistently meet critical deadlines for charting.
  • Skill in self‑managing time and schedule.
  • Demonstrated experience with tablets, mobile phones and EMR software.
