United Health Group is seeking a Hospice Nurse Practitioner to provide consultation in palliative care, symptom management, and supportive care for Hospice patients as requested by the referring physician. You will work within a multidisciplinary team to assess patient needs and inform care decisions.
Primary responsibilities include visiting patients, coordinating care plans with the Hospice Medical Director and other physicians, and documenting patient progress.
