Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and optional diagnostic tests if ordered by the health plan.
You will work with a national network of clinicians, travel locally within licensed states, and conduct virtual visits as needed using an iPad and clinical workflow.
