Advanced Practice Provider (APN/PA) – Primary Care, Family Medicine - Location:
Various locations in northwest suburbs (Arlington Heights, Rolling Meadows, Schaumburg, Mount Prospect, Buffalo Grove, Kildeer). - Full Time / Part Time:
Full Time, 40 hours per week - Hours:
Monday – Friday, hours vary - Saturday rotation and call rotation required

Under the direction of collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures, and interprets results. Instructs patients on the prescribed therapeutic regimen and pre‑/post‑operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting.
Collaborates with physicians and the healthcare team to achieve optimal patient outcomes. Prescribes pharmacologic and non‑pharmacologic therapies as appropriate. Demonstrates evidence‑based practice. Participates in the development, implementation and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain and improve health. Provides comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
Promotes wellness and prevention of illness and injury in various settings for individuals, families and communities. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures.
- License:
Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required. Federal DEA and Illinois Controlled Substance License required. - Education:
Completion of a master’s level program in specialty area. - Certification: BLS/CPR certification required.
- Experience: Minimum 1 year APN/PA experience in Primary Care setting required. 2 years preferred.
- For APNs, Family Nurse Practitioners only.
- Experience working in an outpatient family medicine setting.
- Experience with both acute and chronic condition management and preventative care.
