• Lead and collaborate as case manager to develop and implement patient care plans.
• Provide direct nursing care to patients in the home setting.
• Document care and charting in a timely manner.
• Coordinate with interdisciplinary team and support flexible scheduling.
• Ensure safe and accurate medication administration and supply management.

• RN license in Illinois (or the state of practice).
• Graduate of an approved practical nursing school.
• Minimum one year of RN experience in a clinical setting.
• Current driver’s license and ability to travel approximately 20% of work time.
• Strong time‑management and self‑management skills.
• Proficiency with tablets, mobile phones and EMR software.
