Family Nurse Practitioner (FNP)Position Overview
Family Nurse Practitioners in this role work in close collaboration with a dedicated clinical care team to deliver evidence-based, patient-centered care to a defined panel of patients. The clinical care team partners with the provider on a daily basis to improve health outcomes across the patient population through coordinated, high-quality primary care delivery.
This role reports directly to the practice manager and operates within a structured outpatient clinical environment focused on accessibility, continuity of care, and chronic disease management.
Family Nurse Practitioner (FNP) Schedule3 shifts per week consisting of 10-hour days during the workweek
Plus coverage every other weekend (both Saturday and Sunday required on scheduled weekends)

Saturday: 9:00 AM – 5:30 PM
Sunday: 9:00 AM – 4:30 PM
Family Nurse Practitioner (FNP)Key Responsibilities
- Provide comprehensive Family Medicine and Primary Care services, including assessment, diagnosis, treatment, and ongoing management of acute and chronic conditions
- Conduct routine patient examinations and manage scheduled appointments to support optimal health outcomes
- Develop, implement, and adjust individualized care plans in coordination with the clinical care team
- Order and interpret diagnostic tests as clinically indicated
- Provide patient education focused on preventive care, wellness, lifestyle modification, and chronic disease management
- Support behavioral health screening initiatives and health promotion activities
- Ensure accurate clinical documentation and adherence to quality standards
- Perform administrative clinic duties including opening/closing procedures, inventory management, insurance verification, patient follow-up, and payment collection
- Maintain a clean, safe, and compliant clinical environment
- Collaborate with multidisciplinary healthcare professionals to ensure continuity and coordination of care
- Encourage patient engagement and active participation in health management
- Active Family Nurse Practitioner (FNP) certification required
- Minimum of 1 year recent Primary Care, Family Medicine, or Urgent Care experience required
- Active Illinois Advanced Practice Nurse (APN) license or eligibility to obtain
- Master’s degree from an accredited Family Nurse Practitioner program
- National Board Certification required
- Strong understanding of chronic disease management and quality metrics
- Ability to work independently while functioning within a clinical care team model
- Strong communication skills across verbal, written, and electronic formats
- Excellent organizational skills with ability to manage competing priorities
- Comfort with EMR systems and clinical information technology
- Self-directed with strong problem-solving and decision-making ability
Willingness to obtain multi-state licensure as needed
