Eden Vista Hoffman Estates, part of the Vista Senior Living Management Network, is seeking Wellness Nurses for our wonderful facility!

Wellness Nurse

• Plan and deliver direct and indirect patient care through the nursing process of assessment, planning, prevention, and evaluation.
• Direct the day‑to‑day activities of the nursing assistants.
• Prepare and administer medications as ordered by the physician and perform other drug administration.
• Communicate changes in patient clinical condition with physician, nursing supervisor, and family.
• Complete required record‑keeping upon residents' admission, discharge, and transfer; chart and document per facility policy.

• Customer service orientated.
• Hold a current LPN (Licensed Practical Nurse) or RN (Registered Nurse) license in good standing.
• Have a thorough working knowledge of current care standards and regulations.
• Have experience in the hands‑on care of memory‑impaired residents.
• Can maintain and update effective service plans and supervise the care staff.
• Hold a comprehensive working knowledge of current medication regulations and laws.
• Licensed Practical Nurse (LPN, LPN Nurse, LVN, New Grad LPN) or Nursing student.
• Experience in long‑term care preferred.
• Communicate well in both verbal and written forms.
• Committed to interacting professionally and with care toward residents, families, and other staff.

• Love working with people and handle everyday challenges with a smile.
• Graduate or future graduate from an accredited nursing program.
• Hold a current state license as a Licensed Practical Nurse.
• Communicate well in both verbal and written forms.
• Have a minimum of one year experience as an LPN in a long‑term care setting.
• Be a natural problem solver.
• Be committed to interacting professionally and with care toward residents, families, and other staff.
