The Case Manager Registered Nurse will facilitate the delivery of healthcare services to members, focusing on those enrolled in Medicare and Medicaid. This role is ideal for individuals who are passionate about improving patient outcomes through comprehensive care coordination.

• Travel: 50-75% travel required to meet members face to face in Cook County and surrounding areas

• Act as a liaison with members, employers, providers, and insurance companies.
• Implement and coordinate case management activities for catastrophic cases and chronically ill members.
• Interact with members telephonically or in person, including home visits.
• Assess and analyze members' medical and vocational status; develop care plans.
• Communicate with stakeholders regarding member care and treatment.
• Prepare documentation of case work activities.
• Consult with internal multidisciplinary teams to maximize health outcomes.
• Provide educational and prevention information for best medical outcomes.
• Monitor member progress toward desired outcomes through assessment and evaluation.

• Associates degree in nursing required, bachelor's preferred

• 3-5 years clinical practical experience
• 2-3 years case management, discharge planning, and/or home health care coordination experience

• Active and unrestricted Illinois RN license required
• Certified Case Manager preferred
