Student Services Facilitator serves as the LEA serving two of our K-4 buildings.
QUALIFICATIONS:- Valid Illinois Professional Educator License (PEL) with one of the following:
- LBS I
- School Social Work (SSW) endorsement
- School Counseling endorsement
- School Psychologist endorsement
- Speech Language Pathologist endorsement
- A completed/submitted online application, resume, and letters of recommendation as required on the online application.
- Ability to work as a team member.
- The ability to speak a foreign language is preferred.
- Prepares for and facilitates 504 meetings, special education meetings and other assignments.
- Facilitates and participates in IEP meetings to determine the eligibility and appropriate planning for students who require special education programs and services as the Local Education Representative (LEA).
- Interprets and integrates data and develops school programs to meet the special needs of students.
- Assists in the development of forms and procedures to guide the Individual Education Plan Team process.
- Assists in the review of information and/or completes evaluations of students as needed in the identification of children who are disabled under Section 504 of the Rehabilitation Act of 1973, and assists in developing interventions and/or accommodations as needed.
- Consults and collaborates with parents, teachers and outside agencies in providing services for students.
- Consults with parents, teachers and other staff members in developing and implementing plans.
- Collaborates with community agencies/organizations to connect students with community resources.
- Consults with staff in development and management of special education programs.
- Consults with teachers, parents and/or administrators regarding students transitioning from elementary to middle school and from middle school to high school.
- Consults with the education team to develop appropriate recommendations and programs for students.
- Determines academic, social and behavioral functioning as it pertains to the school environment.
- Documents student contact, including medical assistance (MA) billings.
- Participates in planning and decision-making as a member of Individual Education Plan teams and other decision-making teams as necessary.
- Reviews results of individual student performance at the District's kindergarten screening and communicates results to the child's parents and District Administration.
- Reminds staff of when reevaluations, such as Individual Education Plan assessments, are due.
- Responsible for arriving at multiple worksites which usually means driving between various work sites.
As per current State Law and the District’s current collective bargaining agreement.
