Position: Occupational Therapist (OT) - HomeCare on Fieldstone

Overview

The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages to resume or adjust activities of daily living. Collaborates with the health care team and modifies the plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending upon census and program need.

Responsibilities
  • Review plan of care and orders with primary physician, documented as read-back, completed immediately and transmitted within the time frame established by policy.
  • Educate patients and family members in a manner that overcomes barriers; tailor education to learning capabilities and fundamental needs.
  • Document thoroughly about each patient visit so staff can determine exactly what and how care was rendered with a clear plan for the next contact.
  • Document within guidelines and time frames set by regulatory requirements and/or department policy.
  • Update goals promptly and ensure they are closed as met or changed as the patient condition progresses.
  • Collaborate and initiate case conferencing as necessary; document interactions with other disciplines, MD, care coordinator, insurers, etc.
  • Document care plans and patient/family assessments; demonstrate therapy knowledge, assessment skills, and critical thinking.
  • Anticipate and order an appropriate quantity of supplies with sufficient lead time so the patient does not run out between visits.
  • Educate and document discharge planning prior to discharge, including communication with physician, insurance source, outpatient visits and community resources as applicable; provide the patient with discharge documentation.
  • Complete audits and other assigned tasks within the assigned time frame.
  • Evaluate the patient's level of function within regulatory requirements by applying diagnostic and prognostic procedures as directed by the physician or podiatrist.
  • Guide the patient in the use of therapeutic, creative and self-care activities to improve function.
  • Observe, record and report to the physician or podiatrist the patient's reaction to treatments and any changes in condition.
  • Instruct other health team personnel, including OT Assistants, Home Health Aides and family members in specified phases of occupational therapy.
  • Communicate OT Assistant assignments with scheduler, prepare written instructions for the assistant and supervise the assistant in the patient's home.
  • Understand home care rules and regulations and the impact on case mix index.
  • Complete and document OTA supervisory visits, as required (at least every 6 visits). Complete HCA supervisory visits every 14 days if therapy is the only service in the home.
  • Complete comprehensive OASIS assessments of patients and assess homebound status; conduct comprehensive occupational therapy evaluations in the patient’s home and develop a plan of care.
  • Communicate with patients to schedule appointment times and manage the daily schedule; assess home safety and recommend equipment to improve home safety.
  • Educate family/caregivers on the recommended home exercise program.
Qualifications
  • Licensed Occupational Therapist (OT) - Illinois Department of Financial and Professional Regulation (IDFPR).
  • Proof of Auto Insurance as required by regulations.
  • Basic Life Support (BLS) within 30 days - American Heart Association (AHA).
  • Driver's License - Secretary of State (SOS).
  • Education:

    Bachelor's Degree in Occupational Therapy (Required).
  • Work Experience:
