We are expanding our behavioral health program and are seeking a dedicated Psychiatric Mental Health Nurse Practitioner to provide psychiatric services across long-term care and post-acute facilities. • Conduct comprehensive psychiatric evaluations, including cognitive, emotional, and behavioral assessments
• Provide ongoing psychotropic medication management and monitor treatment effectiveness
• Diagnose and manage a wide range of psychiatric conditions including mood disorders, anxiety disorders, psychotic disorders, and dementia-related behavioral symptoms
• Develop and modify individualized treatment plans based on clinical presentation and medical complexity
• Collaborate with primary care providers, nursing staff, social workers, and facility leadership to ensure coordinated care
• Educate patients, families, and caregiving staff on diagnoses, treatment plans, and medication use
• Respond to acute psychiatric or behavioral changes within the facility
• Participate in interdisciplinary care meetings and contribute to clinical quality initiatives
• Maintain accurate, timely, and compliant clinical documentation
