Registered Nurse; Chicago area and Skokie

Reliable Home Care Providers, Inc. is hiring a RN in the Chicago area and Skokie. Join our growing team of healthcare professionals as a home health nurse.

The RN will provide care in the following areas:

• Skokie
• Rogers Park
• Edgewater
• Lincoln Square
• Lake View
• Lincoln Park
• Downtown Chicago
• Surrounding areas

• Completing the nursing process in the delivery of patient care, including assessment, planning, implementation, and evaluation of patient outcomes
• Total oversight of patients from admission to discharge
• Developing a plan of care based on findings
• Serving as the patient’s advocate and maintaining ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care
• Timely completion of all skilled nursing documentation
• Providing direct supervision of nursing clinical care team members (LPN/LVNs & HHAs)
• Working within a multidisciplinary team approach, coordinating with PT, OT, ST, MSW, HHA’s, and office staff

• $300 sign‑on bonus after 90 days

• Current Illinois State RN License
• CPR Certification
• Previous experience in nursing or other healthcare field experience
• Familiarity with medical software and equipment
• Ability to build rapport with patients
• Strong problem‑solving and critical‑thinking skills
• Ability to thrive in a fast‑paced environment
• Well Sky EMR knowledge is preferred but not required
