MDS Coordinator; LPN or RN

Prairie Oasis is a skilled nursing home facility located in South Holland, IL. We are currently seeking an experienced MDS/Care Plan Coordinator, either an RN or LPN. The MDS Coordinator is responsible for planning, organizing, developing and directing the MDS assessments and care plans in accordance with current applicable federal, state and facility standards, guidelines and regulations. This position is responsible for appropriate reimbursement through accurate completion of the MDS.

This person will also develop, implement and supervise the restorative nursing program and coordinates the corresponding programs.

• Review MDS and care plan data to ensure completion
• Conduct initial assessments and develop, implement, and supervise the restorative program and plans of care
• Review all admission assessments, transfer information and hospital and discharge summaries to determine criteria for Medicare admission
• Responsible for Medicare monitoring admissions and discharges

• Active IL RN or LPN licensure; prefer MDS and rehabilitation credentials
• 2 – 3 years’ applicable experience as RN or LPN and 1-3 years’ experience in MDS; preferred in long-term care
