Position
Full Time Registered Nurse Case Manager – Territory:
West Sacramento & Downtown Sacramento.
Utilize your clinical skills to create, implement, and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
- Perform a full assessment of the patient and family upon initial visit.
- Use assessment data to develop an advanced plan of care including rehabilitation.
- Work with patients and family one‑on‑one to achieve improvement and/or stability.
- Case manage the patient’s care episode with an interdisciplinary clinical team.
- Prepare clinical notes, orders, and charting in a compliant time frame.
- Collaborate with physicians and agency staff to deliver the best care possible.
- Initiate and administer IV medications (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols.
- Current and unrestricted CA RN license.
- Current BLS/CPR certification.
- Valid CA driver’s license.
- High level of clinical knowledge in home health care (highly preferred).
- Experience with high acuity patients (plus).
- Experience working with an interdisciplinary team (highly desired).
- Case management fundamentals (highly desired).
- Previous experience with IV medication infusions (highly preferred).
- Proficiencies initiating intravenous therapy (plus).
- Self‑directed, empathetic, critical thinker, and outstanding interpersonal skills.
- Leadership skills and strong organization.
- Concierge support for case managers to handle administrative tasks.

- Competitive base compensation with an incentive bonus program.
Wage range: $130,000 – $160,000 plus $10,000 annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits, and a 401(k) match of up to 4%.
