Position Overview

The Illinois Department of Public Health is seeking a highly motivated, detail-oriented individual to serve as a Health Facilities Surveillance Nurse in the Division of Field Operations. This individual will conduct surveys of facilities, including state licensure, federal certification and complaint surveys. The HFSN will be responsible for preparing written survey documentation.

Under general supervision, the HFSN will survey facilities to determine compliance with state licensure requirements, federal certification mandates, and Department directives, program policies and procedures. Travelling to conduct routine health and safety surveys to determine compliance with state and federal requirements. Interviews facility residents, staff, administrators, health care providers and other individuals. Observes patient care provided iews medical records, policies, standard operating procedures and other facility documents related to licensure and/or regulatory compliance.
