Community Nursing Support
- Provide onsite nursing support in the Wellness Centers at Addolorata Villa (AM) and Victory Lakes (PM), Monday-Friday, approximately 9:00 a.m.
-5:00 p.m. - Assist residents with their medical care journey, serving as a key point of contact for clinical questions and concerns.
- Support the Care Manager and Care Coordinator with client management, including monitoring status changes and coordinating services.
- Communicate with clients’ doctors, hospice, home health agencies, and pharmacies to coordinate and carry out provider orders.
- Carry out doctors’ orders for clients within scope of practice and agency policy.
- Complete required weekly medication fills for private-pay clients and other requested payable nursing services.
- Ensure all nursing processes and services comply with IDPH regulations and our Home Nursing license requirements.
- Serve as the content expert on our Home Nursing license, answering questions on allowable services and making final decisions on whether requested services fit within licensure guidelines.
- Review nursing services, documentation, and processes for regulatory compliance and quality.
- Perform, upload, and update nursing assessments as needed.
- Complete weekly nursing notes, upload, and update as needed.
- Manage and document medication changes.
- Maintain and update 90-day care plans.
- Ensure completion and tracking of yearly vaccination forms.
- Registered Nurse (RN) with an active Illinois license.
- Experience with geriatrics, senior living, home care, or home health strongly preferred.
- Working knowledge of IDPH regulations for Home Nursing Agencies or willingness to become the internal expert on these standards.
- Strong documentation skills and comfort using electronic systems for assessments, notes, and care plans.
- Excellent communication and collaboration skills with residents, families, physicians, hospice, home health, pharmacy, and internal teams.
- Ability to travel daily between two communities in Lindenhurst and Wheeling, IL
