Graham Healthcare Group is seeking a Home Health Physical Therapist for a job in Wilmette, Illinois.

• Grounded by our belief that outstanding care is best delivered in a team-based environment, our home health physical therapists collaborate to provide management and delivery of patient care plans.
• This full-time position supports patients in the Chicago, Lincolnwood and surrounding areas.

• Eligible for a $20,000 total bonus potential, including a $10,000 first year success bonus and a $10,000 sign-on bonus paid within the first 18 months of employment. Bonuses are contingent on being in good standing, meeting performance expectations, and remaining in the role and status for which you were hired.

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required.
• Licensed Physical Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• A commitment to consistently meet critical deadlines for charting.
• The skills needed to self-manage your time and schedule.
• Demonstrated experience with tablets, mobile phones and EMR software.
